SOUTH BEND OFFICE

From the north:

• Go south on S.R. 933 (Also know as Michigan Street).
  (From the I 80/90 toll road, take exit 77 toward Notre Dame)
  
• Turn right at the stoplight on Bartlett.
  
• Turn left at the first stop sign.
  
• Go past the main entrance to Memorial Hospital, following the signs to Centennial Medical Square and Lafyette Street Parking Garage

From the south:

• Take S.R. 933 (also known as Michigan Street) north to the stoplight at Bartlett Street.
• Turn left at the stoplight on Bartlett.
• Turn left at the first stop sign.
• Go past the main entrance to Memorial Hospital, following the signs to Centennial Medical Square and Lafayette Street Parking Garage.

Alternative from the south:

• Take Lafayette Street north.
  (It is one way until you get through the downtown area and then it becomes two way.)
• Go straight through the stop sign at Navarre.
  (You will be driving along the side of the Lafayette Street Parking Garage, Which is where you want to park)
• Turn right at the first driveway and right again into the parking garage.

Parking:

• Park in the Lafayette Street Parking Garage.
• Take the parking garage elevator to the 5th floor.
• Our office is the first door on the left, Suite 502, Centennial Medical Square.
• Parking is free
• Bring your park card/ticket to our office so we can stamp it for you.

LAPORTE OFFICE

Our office is located in the LaPorte Medical Group Office, 900 I Street.

Directions:

• From Lincolnway (Highway 2), turn south on I Street and proceed about 7 blocks.
• LaPorte Medical Group will be on the right. It is across the street from LaPorte High School.
• Park, enter the main entrance and go to the 2nd floor, and then head to the "Internal Medicine" area and check in with the receptionist.
